Based on 13-month calendar (December 2011 - December 2012) 1-color imprint:
$2.45 per calendar

$75.00 flat setup fee for orders between 100 - 450
$50.00 flat setup fee for orders over 500 and up.

For multiple color imprints:
2 color imprint: add $.60 per calendar
3 color imprint: add $1.00 per calendar
